Thank you for your interest in receiving care at Apex Physicians Infectious Diseases Specialists. We ask that you first call us at 480-454-4185 to explain the reason for the appointment and for us to collect some basic information about you.
In addition, we ask that you provide us with the following important health information:

Recent office notes from your referring physician (the last two visit notes should suffice)

Recent office notes from other providers (if relevant to your referral)
-
Recent lab work related to your illness
-
A copy of the front and back of your insurance card(s)

Once records are received by our office, please allow one to two business days for the new patient coordinator to contact you and set up an appointment.
Once your appointment is confirmed, please download, complete, and bring with you to your appointment (or email us) the New Patient Forms – this will save you time on the day of your visit.
Or, if you prefer, please arrive at your appointment 30 minutes early to complete these forms.
